Here is a risk assessment I have done before making my Music video.  This is useful to make me think about anything which might be a risk, and to think about what I can do to prevent them from happening.

Fire	Actors and director	Building, camera, lens and personal equipment (phones )	Alarm and fire drills/procedures	Very LOW	Make sure no equipment gets too hot and battery are not near anything flammable
Trip over  equipment	Actors and director	Equipment  may be damaged if the person trips over and falls on them	Look for a safe location in the hall to store all equipment	MODERATE	Put all equipment in protective box and make sure wires are taped to the floor
Falling of stage	Actors and director	Equipment could get damaged	Walking carefully around the stage. Placing equipment down in safe places	MODERATE	Put bright colored tape around the edge of the stage
Electric shock from leads and sockets	Actors and director	Fuses and power supply to equpment	Make sure not to touch any exposed wires	LOW	Put signs over areas which could potentially cause electric shocks
